
Individual Appointment Information
Read and download the following documents as part of your enrollment as
an IHSS independent provider
Please remember to BRING YOUR SOCIAL SECURITY CARD to the appointment.
We are required to see and copy your current SSN card and can not proceed without it.
We must also see and copy your un-expired Govt Issued Photo ID.
Your final step will be to go to a LiveScan
location to have your fingerprints taken

- Attention:
Do not enter any live scan facility if you have any COVID-19 symptoms
(fever, cough, trouble breathing, etc.), are awaiting the results of a
COVID-19 test, or have been advised to self-quarantine.
- We can also provide these forms at your individual appointment.
After all the steps are done you will receive a letter confirming your
enrollment is completed.
